For GPs
Referring toLucero Clinic.
An open referral, addressed to the clinic, is all we need. Online appointments in every state and territory except Tasmania and the Northern Territory.
The referral
An open referral is enough.
One 60-minute video appointment with a consultant psychiatrist: mental state examination, diagnostic assessment, treatment and a monitoring plan. The psychiatrist who assesses the patient treats and reviews them.
- Who can refer
- A general practitioner or a nurse practitioner.
- Address it to
- The consultant psychiatrist, Lucero Clinic. A named referral is accepted but not needed.
- Form
- Written, signed and dated. Letterhead, practice software or secure messaging are all fine.
- Include
- Patient name, date of birth and Medicare number; the reason for referral; relevant history, current medicines and any earlier assessments; blood pressure and heart rate if you have them; your provider number.
- Send it
- Give it to the patient to upload with their questionnaire, or email it to clientservices@luceropartners.com.au.
- Valid for
- 12 months from the date you sign it. It must be dated before the appointment.
- What comes back
- A written report and a 12-month management plan, within two weeks of the appointment. You can take over care where the framework allows.
- No referral yet
- Patients can join the wait list without one.
