Before the appointment

The questionnaire does the groundwork.

The questionnaire link arrives the moment you book. It includes the Adult ADHD Self-Report Scale, your history, your health and current medicines, and an identity check. You can upload reports at the same time.

Doing it before the appointment means the 50 minutes are spent on you, not on forms.

Bring your diagnosis letter from a psychiatrist or paediatrician and your current prescription or treatment plan. The GP confirms your treatment, checks your blood pressure and heart rate, and takes over your prescribing. No new assessment.
